Compare full leave, work during Elternzeit where relevant, and the working pattern you would like when you return. Keep one workable fallback.

Separate formal notice from discussion
Identify what must be submitted, when it must arrive and which practical points belong in a meeting rather than the formal notice.

Before coming back, confirm working days, reporting line, responsibilities, access, handover and any phased reorientation the workplace can offer.
